Please note that currently Dr. Farber is seeing patients mostly by virtual visits using video-conferencing
software called vSee Messenger which is a HIPAA compliant
secure software (similar to Zoom or WhatsApp) that you can install on your phone, laptop and desktop (you can visit
www.vSee.com to learn about this software).
Please note that even though most of your sessions with Dr. Farber may be virtual ones via vSee,
if you are using controlled substances such as benzodiazepines, Ambien or other sleeping pills,
or stimulants, you would need to see Dr. Farber in the office intermittently.
Prior to your appointment please complete the
registration form (editable PDF file)
and please send the completed form together with a copy of your insurance card and any photo ID back to Dr.
Farber's office by one of the following ways, whichever is more convenient for you:
1. once you install vsee messenger and connect your vsee account to Dr. Farber's vsee account (you will get instructions on how to do so from Dr. Farber's staff) you can simply send the completed editable
PDF file as attachment on the vSee chat.

2. by fax to Dr. Farber's secure fax number (516) 714-5096
3. If you print the registration form, you can take pictures of the pages with your phone, and text it back to Dr. Farber's office at (516) 714 5098.
Please make sure when taking the pictures you use high resolution settings of your phone camera so the pages would be readable.
If you prefer you can just send the registration form via regular mail to Dr. Farber's office at 295 Northern Boulevard

Please text Dr. Farber's office at (516) 714 5098 once you send the materials to ensure we received it.
As regular email is not HIPAA compliant and not as secure, we do not recommend to send your registration from by email.
