Please call or text Dr. Farber's office first at (516) 714 5098 to find out if Dr. Farber is accepting new
patients at this time.
Please leave a detail message on this confidential voicemail with your name, who
referred you to Dr. Farber, your insurance, and some background information
for the reason for the consultation. Please make sure you leave your cell phone number so we can text you
back.
Please note that currently Dr. Farber is seeing patients mostly for virtual visits using video-conferencing
software called vSee Messenger which is a HIPAA compliant
secure software (similar to Zoom) that you can install on your phone or laptop/desktop (you can visit
www.vSee.com to learn about this software.
Please note that even though most of your sessions with Dr. Farber may be virtual ones via vSee,
if you are using controlled substances such as benzodiazepines, Ambien or other sleeping pills,
or stimulants, you would need to see Dr. Farber in the office intermittently.
Prior to your appointment, please fill out the
registration form
and please send the completed form together with a copy of your insurance card and any photo ID back to Dr.
Farber's office by one of the following ways, whichever is more convenient for you:
1. by fax to Dr. Farber's secure fax number (516) 714 5096
2. once you install vsee messenger and connect your vsee account to Dr. Farber's vsee account, you can send
PDF files or pictures of the pages as
attachments in the vsee chat.

3. take pictures of the pages with your phone, and text it back to Dr. Farber's office at (516) 714 5098.
Please make sure when taking the pictures you use high resolution settings of your phone camera so the pages would be readable.
If you prefer you can just send the registration form via regular mail to Dr. Farber's office at 295 Northern Boulevard
